This is just a short note to explain who we are at Horizon Wealth Management (“Horizon”) and the services we provide to the Company.

Horizon is the appointed Financial Adviser to the Company Superannuation Fund.

Horizon’s role is to provide you with:

  • Education
    This includes education on the features of your Superannuation Fund together with the knowledge that you require in order to maximise the use of superannuation as the primary vehicle for retirement.
  • Advice
    Should you wish to avail yourself of this service, Horizon can assist you with the completion of a personal financial plan.

Horizon has created a checklist to assist you with the administration applicable to your Superannuation Fund.
The intention of this document is to highlight the areas that you may wish to focus your attention on and possibly seek personal financial advice.

CHECKLIST

Step 1. Your Superannuation Fund
Following your first contribution, Colonial First State will send you a Welcome Kit containing your personal investment and insurance details and investor account number. In this Kit, you will also receive information about the range of services that FirstChoice Employer Super provides, including automatic access to the FirstBenefits program.

You will also be sent:

 “Investor PDS (Product Disclosure Statement)” which includes a number of forms, including a “Super Consolidation form” which can be used to consolidate your superannuation into your FirstChoice Employer Super account.
• “New Investor form” which needs to be returned to Colonial First State and also allows you access to online functionality on your account.
 “Beneficiary Nomination form” which is used to assist you with nominating your beneficiaries as well as an “Investment Selection form” which can be used for making your investment selection.

Please Note:

  • Special attention should be given to the completion of the “Investment Selection form” . If this is not completed, your funds will be invested in the default option. The default fund is the Colonial FirstChoice Lifestage Option.
  • Select the level of Life Cover and TPD required. Special attention should be given as the Company has nominated a minimum level of cover. The level of cover is $400,000. You will be required to undergo a medical assessment for a level of cover greater than $400,000. The Company pays the premiums up to a level of cover of $400,000.
  • Select the level of Salary Continuance/Income Protection required. The Company will pay the premiums on your behalf, regardless of your salary. You may be required to undergo a medical assessment should you require a level of cover greater than a benefit of $8,000 per month (which equates to an annual base salary of $114,286 pa)(ex. super).

Step 2. Superannuation Consolidation
Should you have a superannuation account from a previous employer or other superannuation accounts that have been set up during your working career you may wish to consolidate them into the Company Superannuation Fund.

The benefits are:

  • Reduces paperwork – there is less administration hassle in managing a single account,
  • Avoid paying multiple sets of fees, and
  • Easier to control your investments – Your investments should be more effective in one account than in many which means that you will always know exactly how much you have accumulated.

2. Contact your HR department to:

  • Change your personal details
  • Update your salary information to Colonial First State
  • Obtain copies of the Product Disclosure Statement
  • Make salary sacrifice contributions into Superannuation
  • Obtain Personal Statements for Group Insurance – Life,TPD and Salary Continuance where applicable

3. Contact Colonial to:

  • Perform an investment switch
  • Consolidate your super accounts
  • Enquire about account – find out how much you have invested
  • Issue passwords – get started, get online
