To set up a spouse account, please complete the Spouse Application form and send to Colonial First State for processing.

To direct future employer superannuation contributions for your spouse to the Colonial account,

• Download the ATO Super Choice Form, complete Section B and sign the document,
• Print the CFS FirstChoice Employer Super Account Management Guide, and
• Print the CFS FirstChoice Employer Super Compliance Advice.

Please provide all three documents to your spouse’s employer for processing.

To rollover your spouse’s existing super benefits from BT to the new Colonial account, please download and complete the Colonial Super Consolidation form and send it to Colonial First State for processing. Please note that a certified ID document is also required.
